Parent/Guardian Pre-Assessment Questionnaire
Background information on your child, what’s prompted the enquiry, and a history of your child’s difficulties.
SASC Visual questionnaire
This is essential. Your child will also need to have had an eye test in the last two years, but preferably the last six months, prior to having an assessment.
Pupil questionnaire
For your child to complete. Help them with it if they need it.
Checklist and Terms
Our terms of service and a short checklist to confirm we’ve covered everything. This is the final form that needs to be completed.

School / Teacher form
If your child attends a school and you’d like us to liaise with their teacher or SENCO.
Homeschool Parents questionnaire
If you home educate, this replaces the School / Teacher form.
Once you have booked a slot and this has been confirmed, payment will be required to secure the slot.
